Social Prescribing

Social Prescribing is a way for local agencies to refer people to a link worker. Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and well-being. They connect people to community groups and statutory services for practical and emotional support.

The Harrow Partnership involves 5 Harrow Together Members (Age UK Hillingdon, Harrow and Brent, Harrow Carers, St Lukes Hospice, Mind in Harrow, Community Connex), 4 Primary Care Networks(Harrow Collaborative PCN / Harrow East PCN / Health Alliance PCN / Sphere PCN) and Harrow Council Public Health and Central & North West London NHS Foundation Trust.
