Social Prescribing

GPs can refer patients to Social Prescribing Link Worker to help them with their socio-economic needs, loneliness and mental wellbeing by connecting them activities and services in the community. Click here to view online video on ‘What is social prescribing?’.

Upon receiving a referral, SPLW will call the patient to understand support needs and make suggestions/signpost/refer as necessary.  You may contact your GP Practice to connect with your social prescriber. 

The service has a waiting list and waiting time varies from time to time.  You may find list of key services useful and may want to contact them in case you need help sooner.

SAMARITANS:  If you not feeling okay or having a difficult time you can get in touch with them. If you need someone to talk to, they listen. They won’t judge or tell you what to do.  You can talk to them about anything that’s troubling you, no matter how large or small the issue feels.
